Application for Admission
Thank you for your interest in the University of the Incarnate Word School of Optometry (UIWSO).
The School of Optometry offers a program leading to the Doctor of Optometry (O.D.) degree. The School selects students based on the candidate’s application content, pre-professional academic performance, scores from the Optometry Admission Test (OAT), letters of evaluation, personal statements, and the interview.
To apply to the UIW School of Optometry, the applicant must complete the:
- Centralized OptomCAS application: www.optomcas.org

*You must arrange for your official Optometry Admission Test scores to be sent directly to UIWSO Optometry Admission Test
Instructions for the UIWSO Supplemental Form for Admission
Please follow the following steps:
Step 1: Follow the directions outlined on the Supplemental Form, and complete the form in its entirety.
Step 2: After completing the Supplemental Form with an electronic signature, save an electronic copy, with your name in the title.
- For example: UIWSOSupplementalForm_SallieSmith
- It is imperative that your name be attached to the electronic supplemental form as shown in the example above, prior to uploading it, in order for us to identify it as your form. Without your name attached to the file, it is likely that your online Supplemental Form may not be processed.
Step 3: Upload your Supplemental Form to: https://uiwso.box.net/drop/UIWSO/422f766275
It is your responsibility to inform the UIWSO Office of Admissions of any changes in address or telephone number. Questions pertaining to the application process may be directed to the School of Optometry Office of Admissions at (210) 883-1190.
